My initial suggestions would be: - what do you actually want to find out? You'll only get decent results from a decently constructed and decently distributed survey. - how do you intend distributing it? If it only goes through the clubs, BUF, online, etc, you'll end up with a process of candidate self-selection. It might give you a very specific picture of the state of Ultimate at the moment, but won't tell you anything about why people leave the sport. - what do you intend doing with it? Will the survey actually be used to alter/influence anything? Having a picture of Ultimate in 2000 might be an attractive idea on paper, but are any of the clubs or BUF in a position to change the organisation, motivation, recruitment, etc? I don't want to sound depressing, but if you don't intend using the results of a survey, don't actually do one. Thought, suggestions, musings, etc....?
